Goal: The mission of Project Homeless Connect (PHC) is to connect homeless San Franciscans with the care they need to move forward. The goals of Project Homeless Connect are to:

- Improve access to services for homeless San Franciscans.
- Improve the system of care through collaboration and the sharing of best practices among San Francisco’s homeless service provider community.
- Engage and increase the involvement of the business, nonprofit community, and individual volunteers to work together to provide access to services for homeless San Franciscans.

Goal: The goal of this program is to improve health outcomes for pregnant and postpartum women with substance abuse problems and their infants.

Impact: Improvement in birth weight and gestational age, and reduction in admittance to neonatal intensive care unit and positive infant toxicology screens.

Goal: The program’s goal is to delay the age when young people begin drinking and to reduce drinking among those who have already started.

Impact: Studies have shown that by the end of the intervention, participating students were significantly less likely to drink alcohol than nonparticipants. Also, students who did not use alcohol before participating in the program were less likely to use alcohol after the intervention than similar youth who did not participate.

Goal: The goal of the study was to evaluate a community-based food support intervention in the San Francisco Bay Area for people living with HIV and/or type 2 diabetes mellitus (T2DM) to determine the feasibility, acceptability, and potential impact of the intervention on nutritional, mental health, disease management, healthcare utilization, and physical health outcomes.

Impact: Comprehensive, medically appropriate food support is feasible and may improve multiple health outcomes for food-insecure individuals living with chronic health conditions.

Goal: The goal of Project PREVENT is to reduce behavioral risk factors for colorectal cancer among individuals with positive screenings.

Impact: A significantly greater proportion of Project PREVENT participants reduced their multiple risk factor score when compared to the control group (47% vs. 35%). Intervention participants also had significantly greater multivitamin intake and significantly reduced red meat consumption.
