Goal: Vallejo Sanitation's goals in 2001 were to seek the most cost effective treatment of wastewater and to find financial rebates/grants where possible to implement cost saving measures. In order to qualify for generator replacement rebates, the agency needed to design and complete the project in a short time.

Goal: White Crane's health outreach model strives to reduce the incidence of risk factors for preventable disease, to motivate and educate seniors to take control of their health, and to make primary health care services accessible to vulnerable elders.

Goal: To assess the impact of affordable housing on health care outcomes in a low-income population who have experienced housing instability.

Impact: Costs to health care systems were lower after people moved into affordable housing. Primary care visits went up after move-in; emergency department visits went down. Residents reported that access to care and quality of care improved after moving into housing.

Goal: The initiative's primary purpose was to reduce infant mortality by 50 percent and generally improve maternal and infant health in at-risk communities.

Impact: 20% of the Healthy Start program sites had significantly lower rates of low-birth-weight babies than their comparisons. 20% of the sites also had significantly lower rates of very-low-birth-weight babies than their comparisons. Four of the sites had significantly lower pre-term birth rates.

Impact: The Community Preventive Services Task Force (CPSTF) recommends tailored pharmacy-based adherence interventions for cardiovascular disease prevention. Evidence shows interventions delivered by pharmacists in community and health system pharmacies increased the proportion of patients who reported taking medications as prescribed. The CPSTF also finds these interventions are cost-effective for cardiovascular disease prevention.

Goal: The goal of this intervention was to reduce self-reported unprotected sexual intercourse among African American and Latino adolescent girls.

Impact: Skill-based HIV/STD interventions can improve condom-use and reduce sexual risk behaviors, along with STD rates, among African American and Latino teen girls in clinical settings.

Goal: The goal of this program is to end homelessness in the City of Portland and Multnomah County by 2015. As efforts to end homelessness continue, city and county officials will focus on nine actions. Programs throughout the county will address moving people into Housing First, ending the practice of discharging people into homelessness from jails and hospitals, improving outreach, emphasizing permanent solutions, increasing the housing supply, creating new partnerships, improving the rent assistance system, increasing economic opportunity for homeless people and implementing new data collection technology.
